The Virginia Department of Health outlines immunization requirements for schools in Virginia in accordance with šŸŒVirginia Code Ā§ 22.1-270, Ā§ 32.1-46.

Newly Enrolled Students

Families enrolling in Chesapeake Public Schools must provide documentation of compliance with the Code of Virginia for physical examinations. The school physical form is provided by the Chesapeake Health Department, or online at the Virginia Department of Educationā€™s website. This form should be completed by your childā€™s physician and should include proof of adequate age-appropriate immunizations.

Immunization Requirements for students in grade 7 and 12

In the state of Virginia, three vaccines are required for all students entering grade 7: 

  1. Tetanus, diphtheria, and pertussis (Tdap), 

  2. Meningococcal ACWY (MenACWY), and

  3. Human Papillomavirus (HPV)* (dose 1) 

Two vaccines are required for all students entering grade 12: 

  1. A single dose of the Meningococcal ACWY vaccine (MenACWY) (administered after the studentā€™s 16th birthday). 

  2. Human Papillomavirus (HPV)* (dose 2) 

*Note: After reviewing educational materials approved by the Board of Health, the parent/guardian, at their sole discretion, may elect for the child not to receive the HPV vaccine. VA Code 32.1-46.
