Attendance Waiver Request Form
Any student who has more than 9 absences in a semester course or more than 18 in a full-year course should apply for an Attendance Waiver.
Waivers will be granted based on the following criteria:
A. Transfer student to or from a division with differing attendance or clock hour requirements.
B. Documented family or medical extenuating circumstances.
When you complete the form, please attach any appropriate documentation (i.e. physician statement) not already provided to the school at the time of the absence(s).
Please note: Waivers will only be granted if the student has demonstrated mastery of course standards and competencies by earning a passing final grade for the course(s), as demonstrated by course performance and end-of-course summative assessment(s).

Sports Physical Opportunity

Student Schedules
Student Schedules for the second semester will be available on StudentVUE and ParentVUE prior to the first day of the new semester. Any student who wishes to request a schedule change must complete the Schedule Change Form and submit it to their School Counselor.

Seniors – Important
Student Online Verification
At the beginning of every school year, all families must complete the student online verification process. Families must log onto their ParentVUE account to verify their student’s information. The verification process allows our families to confirm or update information for all children in a household at one time. For more information, please visit the CPS Student Online Verification Process webpage.
Families will also have the opportunity to review and update their contact information to ensure continued access to important information and communications from their student’s school and the district.
The deadline for the student verification process is Thursday, September 30, 2021. To complete the process, please log onto your ParentVUE account. The verification process can be completed in ParentVue on web browser or in the ParentVUE mobile app. Once logged in, select ONLINE REGISTRATION & VERIFICATION and follow the prompts to verify your student’s information.
Parent Notifications in Schoology
If any families are concerned that their students are missing assignments, they can set their notifications so that they are alerted when assignments are missing. You can provide them these steps.
- Log onto your Schoology Parent Account.
- Click on “Account Settings.”
- Select “Notifications.”
- Select “YES”next to “Course Materials Overdue” for email or phone.
- Click “Save Changes.”
