Our district Health Services Department is working with the Chesapeake Health Department to offer immunization clinics for families who need to meet the state requirement for rising 7th and 12th graders.
This clinic, hosted at Oscar Smith High School, is open to any rising 7th or 12th grader in our district.
Families with 7th graders should bring
Completed Consent form (Formulario de Consentimiento de Vacunacion)
Immunization record (required)
If you have insurance, please bring a copy of your insurance card

NOTICES OF NONCOMPLIANCE
Families with students entering 7th and 12th grade must submit documentation of required immunizations before the first day of class or the student will not be permitted to attend.
The district and each middle and high school have been sending emails and text messages to families who have 7th or 12th graders who do not have the required documentation on file. We encourage families to submit documentation as soon as possible through our secure, online form: Online Submission Form: Student Immunization Record.
