We are proud to partner with Rite Aid and the Chesapeake Health Department to host vaccination clinics offering the COVID-19 vaccine and the booster shot for those who are eligible, ages 5 and up (schedule available below). Additional vaccines may be available at each clinic. Read more about the CDC recommendation to vaccinate 5-11 year olds.

Participation in these clinics is voluntary. All participants must adhere to COVID-19 mitigation protocols including wearing a mask, maintaining six feet physical distance, and completing temperature screening upon arrival.

The Pfizer COVID-19 Vaccine is administered in two doses issued 21 days apart. The vaccine will be available for students 12 and older, parents, and employees. Students between ages 12 – 17 must have a parent or guardian present.


All participants  receiving their FIRST DOSE* must bring the following information with them:

(1) Completed Forms (Insurance and Consent) (Los documentos en espanol)
Additional copies of all required forms will be available upon request at each clinic.

(2) A photocopy of the front and back of the following documents:

  • Medical Insurance card**
  • Prescription Card
  • Government issued photo id

*If you are receiving a second dose, please read our FAQ to determine what paperwork you will need for your appointment.

If you do not have insurance, you will need to provide your social security number at the time of service. There is no charge to the patient or the district for the COVID-19 vaccine but we are required to gather this information per the Department of Health. No one will be denied the vaccine if not insured.


FREQUENTLY ASKED QUESTIONS

Which vaccine will be offered at this clinic?

Rite Aid is administering the Pfizer vaccine, which is currently approved for individuals 12 and older and is administered in two doses. The second dose must be administered 21 days or more after the first dose. Every individual who registers for an appointment at this first dose clinic will automatically be registered for their second dose at the same location.  

I missed my second dose appointment from a previous clinic. Can I show up to this clinic to get my second dose?

Rite Aid will be offering both first and second dose vaccines on a first come, first served basis at these clinics. Please call Health Services at 494-7628 to secure your second dose appointment.

What do I need to bring to my appointment?

IF YOU ARE RECEIVING YOUR FIRST DOSE
  • Rite Aid Paperwork (all three pages completed)
  • A photocopy of the front & back of the following documents:
    • Medical Insurance Card
    • Prescription Card
    • Government issued photo ID
IF YOU ARE RECEIVING YOUR SECOND DOSE (and your first dose was administered at a RiteAid supported clinic)
  • Rite Aid Paperwork (first two pages completed)
  • Vaccine Card (issued at first dose appointment)
IF YOU ARE RECEIVING YOUR SECOND DOSE (and your first dose was NOT administered at a RiteAid supported clinic)
  • Rite Aid Paperwork (all three pages completed)
  • Vaccine Card (issued at first dose appointment)

Do I have to schedule an appointment to receive a shot?

Walk-ins will be admitted to the clinic on a space available basis. We strongly recommend that you secure an appointment. Please call Health Services at 494-7628 to secure your second dose appointment.
