Update as of May 13, 2021:  Blue Cross Blue Shield (Anthem) Settlement Notice

You may have received a message from Blue Cross Blue Shield in reference to a settlement notice last month. This is a legitimate settlement case for certain Anthem members covered by Blue Cross Blue Shield health insurance plans during the timeframe of February 2008 through October 2020.

Next Steps:  To see if you qualify for settlement, please file a claim online:  https://secure.bcbssettlement.com/. The deadline to submit claims is November 5, 2021.

If you do nothing, you will remain a member of the Settlement Classes; however, if you are entitled to share in the Settlement proceeds, you will not get a payment.

Claim timeframe:  this case may include eligible coverage/plans back to February 2008. So, we encourage you to locate historical information prior to beginning the claim. For example, you may have been covered by an Anthem plan at a previous employer within the last 13 years. If you have been with CPS for most of the duration, we have historical group numbers here.

Limited fields are actually required (noted below) in the plan entry portion of the claim; however, providing as much information as possible will assist the claims administrator with processing your claim.

Information needed to file your claim:

  • Unique ID:

    • You will be asked for a Unique ID (ten-digit code) which was provided in your email notice.

    • However, if you no longer have your email notice, or if you did not receive one, you may still file a claim online without it.

  • Health Plan Name:

    • For CPS Anthem plans, you should select:  Anthem Health Plans of Virginia d/b/a Anthem Blue Cross Blue Shield (of Virginia).

    • However, if you were covered by another Anthem plan in another state perhaps, please select the applicable plan in the drop down menu in the claim form.

    • Health and Dental plans should be included and you can add as many plan entries as needed online in the form.

  • Anthem Group Number(s):

Submitting your claim:

You are able to submit your claim online OR by mail. All claim forms must be filed online or postmarked by November 5, 2021.

Medical Benefits
Chesapeake Public Schools (CPS) offers eligible employees a choice of medical insurance plans administered by Anthem.  Medical plans cover physician visits, therapeutic services, prescription drugs, vision, and more.

 

Dental Benefits
The CPS dental plan is administered by Anthem and there is no premium for an Employee Only plan.  It covers preventive care like regular cleanings, exams and X-rays, and also procedures like fillings and crowns.

 

Flexible Spending Accounts (FSA)
Flexible Benefit Administrators administers three types of Flexible Spending Accounts (FSA) for CPS.  These accounts can save money by paying for the eligible health and dependent care with pretax dollars.

Legal Resources Identity Protection plan continuously monitors your records for potential threats to your identity and offers assistance if you become a victim of identity theft.

 

Legal Resources
The Legal Plan provides 100% coverage on a broad range of legal services.  Whether your legal matter is expected or unexpected, you’ll have immediate and ongoing access to our network of highly-rated   law firms.

 

Life Insurance
All eligible employees receive basic life benefits through the Virginia Retirement System (VRS).   You may elect to increase your life insurance coverage through VRS or by applying for optional coverage with Anthem.
