ELIGIBILITY: Chesapeake Public Schools (CPS) employees, retirees, and dependents who have diabetes and are covered under CPS health insurance plan.
Participation is voluntary and those participating will actively engage in an educational/health monitoring program and work with a Health Coach to learn how to self-manage their condition. The desired goal of this program is to demonstrate that individuals who participate in the program can learn how to successfully self-manage their own diabetes treatment, and by doing so, improve and maintain their good health while reducing the need for expensive medical services.
INCENTIVE: For participation in this program, CPS will make arrangements to waive the co-pays on purchases of diabetes medications and related supplies. In addition to this immediate cost-saving, participants will also benefit from the health care services that will be provided to them by their Health Coach.
